Please download the necessary tax exemption forms for your state here, complete them, and return them to Lesman's accounting department.

You can e-mail the completed form to ar@lesman.com, fax it to 630-595-2386, or send it by US Postal mail to Lesman Instrument Company, 135 Bernice Drive, Bensenville IL 60106.

In order to comply with state tax codes, we are required to have complete tax information on file for our customers who request Tax Exempt status. All forms below are in Adobe PDF format.
